(1) AFM Neighbouring Rights - General Information
Please view this document. Download and/or print it if you wish to retain a copy. (recommended)

(2) AFM Appointment and Authorization Form
Please view and fill in all required form fields on-line. Print the completed form, sign it and mail it to the address on the form.

(3) AFM Repertoire Registration/Additions Form
Please view and fill in all required/appropriate form fields on-line. Print the completed form (legal size if possible), initial it and mail it to the address on the form.

Note: The forms contain "fillable" fields. Simply click in a fillable area and enter your data while on-line. Print the form when completed. This will greatly assist us to process your information accurately. Upon receipt of your forms, we will provide you with a Username and Password to our secure/private Assignor's area. This area will provide for "real time" on-line updates to our database including: (i)The ability to update your personal profile. (ii)Repertoire additions and corrections. (iii)Searching and viewing of current eligible sound recordings.
