(1) AFM Neighbouring Rights - General Information
Please view this document. Download and/or print it if you wish to retain a copy. (recommended)

(2) AFM Appointment and Authorization Form
Please view and fill in all required form fields on-line. Print the completed form, sign it and mail it to the address on the form.

(3) AFM Repertoire Registration/Additions Form
This form can be completed electronically or it can be printed and completed by hand - the only thing we ask is that you complete the form to the best of your knowledge.  In addition to, or in lieu of this form, some performers submit copies of CD's, copies of liner notes, recording contracts and/or personal databases.  If you do submit an alternate form of repertoire, please ensure that it provides the same information requested in our Repertoire Form.

Note: The forms can be completed electronically. Simply click in a fillable area and enter your data while on-line. Print the form when completed. This will greatly assist us to process your information accurately. Upon receipt of your forms, we will provide you with a Username and Password to our secure/private Assignor's area. This area will provide for "real time" on-line updates to our database including: (i)The ability to update your personal profile. (ii)Repertoire additions and corrections. (iii)Searching and viewing of current eligible sound recordings.
